Basic Level Visit 

Our Basic Package begins with

  • Office exam
  • Up to two labs
  • Up to two injectable medicines
  • One oral medicine


Moderate Level Visit 

Our Moderate Package goes a step further with your care. This package includes:

  • Office exam
  • Up to six labs
  • Up to three injectable or oral medications
  • One office procedure:
    • Simple DME
    • Non-Fracture care


Extensive Level Visit 

This Extensive Package includes:

  • Office exam
  • Up to eight total labs
  • Up to four injectable or oral medications
  • Up to two office procedures
    • Complex DME
